Archdiocese Common Evaluation Forms (below)
Parents fill out the top half of the form and submit it to the student's current school. The school will then share it with CSHM directly. Incoming Freshmen need only ONE evaluation from ether their current Math Teacher, English Teacher OR Current Principal.

Transfer students will need to have three evaluation forms completed from their Math and English teacher and either a counselor or administrator. Freshmen and transfer evaluation forms can be sent via email to various evaluators via the checklist item on the admissions parent portal.

Please fill out the parent permission form on the admissions parent portal so we can request transcript and test scores from your student’s school.

High School Placement Test
All incoming freshmen are required to take the HSPT to be considered for acceptance to the school. CSHM offers the test twice in the spring semester and accepts HSPT scores from any location the student may take the test.  Students are automatically registered for CSHM's test dates with the submission of an application.  Please note, you only need to take the HSPT once, we offer two testing dates for your convenience.
